Question: What documents should I gather for the home study?
Answer:

Each packet is different, as they vary from state to state, but some may include:

    Marriage certificate (if applicable)
    birth certificate
    Physical from doctor or at least a Tuberculosis (TB) test or chest x-ray.
    Financial informaiton: paycheck stubs, W-4, income tax forms
    Insurance policies and coverage
    Names, addresses, and phone numbers of three or four good references
