Question: What documents should I gather for the home study?
Answer:
Each packet is different, as they vary from state to state, but some may include:
- Marriage certificate (if applicable)
- birth certificate
- Physical from doctor or at least a Tuberculosis (TB) test or chest x-ray.
- Financial informaiton: paycheck stubs, W-4, income tax forms
- Insurance policies and coverage
- Names, addresses, and phone numbers of three or four good references
